The Records Office has revamped the Major/Minor/Certificate/Advisor Change Form. The form is available on the Records website under “Forms.”
Please use the new form effective immediately and discard any old forms you have in your departments. Students will need to be directed to the website to print out the form. After it is completed and the necessary signatures are obtained, it must be submitted to the Records Office to be processed.
