MSUM Sexual Violence Reporting Form

Posted on September 12, 2017

Sexual violence is an intolerable intrusion into the most personal and private rights of an individual, and is prohibited at Minnesota State University Moorhead (MSUM). We are committed to eliminating sexual violence in all forms and will take appropriate remedial action against any individual found responsible for acts in violation of this policy.

To further its commitment against sexual violence, MSUM provides reporting options, an investigative and disciplinary process, and prevention training and other related services as appropriate.

One way individuals may make a report of Sexual Violence, including rape and sexual assault, to MSUM is through an online form located on the university Title IX webpage.

The form provides important information for anyone considering reporting, including uniform amnesty and the right to submit the form anonymously. The form is located here: https://cm.maxient.com/reportingform.php?MSUMoorhead&layout_id=20.
