It’s that time of year again! The annual Flu Shot Clinic has been scheduled.

• Wednesday, October 14, 2020, from 7 a.m. – 2 p.m.
More Information
• “Walk-in” basis; no appointment necessary.
• Consider completing the vaccination consent form in advance if possible. Otherwise, printed copies will also be available when you arrive.
• No cost with your Minnesota Advantage Health Plan member ID card. Employees without a valid card or who don’t have health insurance through the State of Minnesota can receive the flu shot for $33.
• Flu vaccinations are also available at your primary care clinic and in-network convenient clinics if you aren’t able to attend on Wednesday.

Please note: Employees who participate in the flu shot clinic will earn 100 points towards our wellbeing incentive program with Virgin Pulse.
