MSUM is collecting emergency contact information in the event you are involved in a life-threatening situation or emergency. By providing an emergency contact, MSUM officials can contact these people on your behalf.

• Your emergency contact should know your medical history and be able to make decisions on your behalf.
• It’s valuable to have several emergency contacts, with multiple ways to contact each one.
• This information is completely confidential and will only be used in an emergency.
