Check your mail for IRS Forms 1095-C
Check your home mailbox! This week (March 23 – 28) Minnesota Management and Budget is mailing over 60,000 IRS Forms 1095-C to employees. As a large employer, the state is required by the IRS to send you this form. The form gives the IRS information about the state’s offer of health insurance coverage to employees. You do not need the 1095-C to file your taxes but you should keep it with your other important tax documents.
- Who receives the form? Any current or former employee who was full-time or was offered health coverage during 2015.
- Where was it mailed? To your home.
- When will I get it? By March 31.
- Can I find my form in Self Service? No. We mailed forms to your home.
