– Life Insurance* (Employee, Spouse, Child)
– Short Term Disability*
– Accidental Death & Dismemberment (AD&D)
– Long Term Disability – Enrollment only allowed during the Annual Open Enrollment Period
*Note: Evidence of good health is required to add Life Insurance and Short Term Disability coverage.
