LASC Assessment forms for Fall, 2015 classes are due Dec 18th.
Please use the electronic form found here. Remember that multiple sections may be pooled into a single report (although this is best done when all sections use the same or very similar assessments) and that if sections are taught during both Fall 2015 and Spring 2016, a single form may be submitted at the end of the Spring, 2016 semester.
