Be sure to check your Primary Care Clinic, as cost levels can change each year.  If you will retain your current medical carrier (BlueCross, Health Partners or PreferredOne) you can change clinics by calling the customer service number on the back of the card (do not submit a clinic change form on Employee Self-Service).  The link to the Primary Care Clinic Directory is mn.gov/insdir/provider_directory_openenrollment.aspx.

Elections must be made by midnight on November 9, 2015.

Information on the 2016 Open Enrollment is posted on the SEGIP website.  The 2016 Open Enrollment Guide will walk you through detailed information on this year’s changes and benefits that are open.  Please take the opportunity to review the guide prior to completing your Open Enrollment.
