All MSUM Records Office forms are now available online only

All MSUM Records Office forms are now available online only. They can be found on the Records Office webpage and the student will be responsible for printing out the form and obtaining signatures. Most forms require the signature of the student and someone from the department of their major (department chair, advisor, dean, etc.)

Forms that can be found on the webpage include, but are not limited to:
1.       Major/Minor/Advisor Change
2.       Excess Credit
3.       Tri-college registration
4.       Academic Appeal
5.       Application for Graduation
6.       Course Exception
7.       Privacy release
8.       Registration permit
